New Zealand’s Department Of Conservation (DOC) has produced standardised forms that must be used when implementing the ‘2013 Code of Conduct for Minimising Acoustic Disturbance to Marine Mammals from Seismic Survey Operations’. These forms can be downloaded from the DOC website (http://www.doc.govt.nz/conservation/marine-and-coastal/seismic-surveys-code-of-conduct/notification-and-reporting/). Note that the forms cannot be used on Apple computers, and macros must be enabled.
There are two nearly identical reporting forms, one for ‘on survey’ and the other for ‘off survey’. The on survey forms are used when the vessel is in the operational area, as set out in the Marine Mammal Impact Assessment (MMIA: www.marinemammalimpactassessment.co.nz).
Each Excel workbook has multiple tabs. How to complete and use each tab is explained within each of the headings shown in the menu above.
The blue tabs require data entry, whilst the yellow tabs are guides or summaries to help observers fill out forms and generate reports.
Within the tabs all cells in white require data entry, but coloured cells are filled automatically, based on what has been entered elsewhere. Do not attempt to alter data in coloured cells.
